For people with dementia, studies have shown that the greater number of stimulating cognitive activities (writing, reading, having group discussions, doing crossword puzzles, and playing music) they are engaged in, the longer rapid memory loss is delayed. Studies have also shown that exercise increases fitness, physical function, cognitive function, and positive behaviour in individuals with dementia and other mild cognitive impairments.
*All services are one-on-one, confidential, person centred and offered in the comfort of the individual's home
Cognitive Coaching and Cognitive Exercises
- Designed to exercise the mind to maintain brain fitness
- Development of a positive, supportive relationship
- When offered on a regularly scheduled basis, these sessions become a highlight for the individual who feels better knowing they are actively working to improve their brain function
Care Management
- Developing a care plan tailored to each individual senior, that is continuously monitored and modified as needed
Transportation and Advocacy for Medical Appointments
- Preparation before each appointment and help to make sure all questions are asked and responses are recorded, if appropriate
Family Updates
- Keeping the family informed and involved through regular phone calls, emails or texts via their preferred method of communication
Regular visits and support will alleviate potential feelings of isolation
- Some times all an individual requires is someone to listen on a regular basis
Walking for exercise
- Physical exercise is important to brain health – improves blood flow to the brain
Computer related communication and assistance
- Helping seniors connect with far away family members through Skype or Face time
- Computer lessons
Creation of a Memory Book
- A great tool to have to promote communication, memories and positive responses
Referral to professional services (nutritionist, personal care, massage therapist)
- Professionals that you can trust to have your loved ones best interest in mind
Referral to other programs that promote community involvement and connection
- Volunteering, adult day support programs, Memory Plus, etc.
Assistance with finding housing and other resources based specifically on the needs of each senior
- Help navigating seniors housing and the resources to make the right decision for your loved one
Cognitive Care Packages
All packages include the following:
- Free initial assessment
- Travel to the home or residence of the individual receiving services
- Updates and concerns communicated regularly to family members
- Tools/Resources required for cognitive stimulation listed under our services
- Monthly MMSE (Mini Mental State Exam) score
Cognitive Package I
Cognitive Package II
- 2 hours a week
- Visits can be 2 consecutive hours or 2 individual visits of 1 hour each
Cognitive Package III
- 3 hours a week
- Visits can be completed in several combinations of 1, 2, or 3 hour segments throughout the week.
POM Package (Peace of Mind)
- Visits defined based on initial assessment and at the request of the family/individual
- Suited for families who want the assurance that their loved ones are thriving.
Visits can be daily, weekly or monthly
